We treat ingrown toenails under local anesthesia in a 20-minute walk-in visit: we remove the edge of the nail that digs in and, if it keeps coming back, apply a treatment so it doesn't regrow on that side. How do I know I have an ingrown toenail?

Pain at the edge of the big toe nail when walking or in shoes, redness, swelling and, once infected, pus and red tissue growing over the nail. The most common causes are cutting the nail too short or rounded, tight shoes, sweating and injuries.

When should I come in?

If pain does not improve in 3 days with warm soaks, if there is pus or spreading redness, if it recurs more than twice, or whenever you have diabetes or poor circulation: in those cases an ingrown nail can get complicated and should not be treated at home.

How is it done?

1. Local anesthesia at the base of the toe; you only feel two pinches.
2. We remove the side portion of the nail that digs in (partial avulsion). The nail keeps its shape.
3. If it is the second time or more, we apply phenol to the root of that edge (matrixectomy) so it does not regrow there; it prevents recurrence in over 90% of cases.
4. Dressing, written instructions and antibiotics if there is infection.

You walk out in an open shoe and can return to your routine in 1 to 2 days.

How do I care for the toe afterwards?

Warm salt-water soaks for 10 minutes, twice a day, for a week; a clean dressing daily; wide shoes for 2 weeks. With phenol, the edge may ooze clear fluid for 2 to 4 weeks; that is normal.

How do I keep it from coming back?

Cut the nail straight across, without rounding the corners or cutting shorter than the tip of the toe; wear shoes with room in the toe box; keep your feet dry.
