How do I know if I have diabetes, high blood pressure or high cholesterol?

They often cause no symptoms until damage is done. We use the diagnostic values from U.S. medical guidelines: fasting glucose of 126 mg/dL or more, or A1C of 6.5% or more, indicates diabetes; blood pressure of 130/80 or more at two visits indicates hypertension; LDL cholesterol above 100 to 160 mg/dL, depending on your risk, needs treatment. A blood test and a blood pressure reading are enough to find out.

What does the care include?

  • First visit: history, physical exam, blood tests (glucose, A1C, lipids, kidney) and a urine test for protein.
  • Treatment plan: affordable generic medications, personal goals for glucose, blood pressure and cholesterol, and an eating plan adapted to Latino food.
  • Follow-up visits every 3 months with A1C and dose adjustment; every 6 to 12 months, a lipid panel and kidney function.
  • Yearly foot check and referral for a retina exam in patients with diabetes.
  • Prescriptions and refills: call ahead to check availability and avoid a full visit every time.

What are the treatment goals?

For most adults: A1C below 7%, blood pressure below 130/80 and LDL below 100 mg/dL, or below 70 if you have already had a heart attack or stroke. The doctor adjusts the goals to your age and other conditions.

What happens if I don't manage them?

Poorly controlled diabetes and high blood pressure damage the heart, kidneys, eyes and foot nerves over the years. Most of that damage is prevented with steady control, and many patients cut back on medication as their diet and weight improve.
