Which symptoms point to a thyroid problem?

• Underactive thyroid (hypothyroidism): constant tiredness, weight gain without diet changes, feeling cold, dry skin, hair loss, constipation, depression and heavy periods. It is the most common form, especially in women over 40 and after pregnancy.
• Overactive thyroid (hyperthyroidism): weight loss, palpitations, nervousness, hand tremor, feeling hot, insomnia and bulging eyes.

A lump in the neck or trouble swallowing calls for a thyroid ultrasound, which we also do in the clinic.

Which tests do we run?

1. TSH: the main screening test. Values above 4.5 mIU/L suggest hypothyroidism and below 0.4 hyperthyroidism.
2. Free T4 and T3: confirm the diagnosis and measure severity.
3. Anti-TPO antibodies: when Hashimoto's thyroiditis, the most common cause of hypothyroidism, is suspected.
4. Thyroid ultrasound: if there are nodules or an enlarged gland.

No fasting needed. We call you as soon as the TSH result is ready.

What is the treatment?

Hypothyroidism is treated with levothyroxine, one daily pill on an empty stomach, 30 to 60 minutes before breakfast and away from calcium and iron. The dose is adjusted with a follow-up TSH every 6 to 8 weeks until you reach the goal, then every 6 to 12 months. Hyperthyroidism is treated with methimazole and checks every 4 to 6 weeks; complex cases are referred to endocrinology.

Should I check my thyroid if I'm pregnant?

Yes, if you have symptoms, family history or already take levothyroxine: during pregnancy the dose usually needs a 25 to 30% increase and checks every 4 weeks. We also recommend the test for women with infertility or repeated miscarriages.
