School & Sports Physical Exams

We do physicals for school, sports, camps and daycare without an appointment, in 20 to 30 minutes, and complete and sign the form your institution requires. At Clínica Hispana Nueva Salud Lawndale we see children and teens in Spanish or English, Monday to Sunday from 9 AM to 9 PM.

Which form do I need?

• Texas school sports (UIL): the UIL pre-participation physical evaluation form, required every year to play sports in grades 7 through 12. It must be signed by a physician, physician assistant or nurse practitioner.
• School, daycare or Head Start: the institution's physical exam form plus the immunization record.
• Camps, leagues and summer jobs: each program's own form.

Bring the form printed or on your phone; if your school did not give you one, we provide a general medical certificate.

What does the exam check?

1. Weight, height, body mass index and blood pressure.
2. Vision and hearing.
3. Heart and lungs, with questions about fainting or chest pain during exercise.
4. Abdomen, spine, joints and muscle strength.
5. History of injuries, concussions, asthma, allergies and medications.

If we find a heart murmur, high blood pressure or an injury that has not healed, we tell you which test is needed before clearing the student for sports.

What should I bring?

• Parent or guardian ID and the student's ID
• School or team form, with the history section already filled in by the parents
• Vaccination card or ImmTrac2 record
• Glasses, if worn, and list of medications

Does it include vaccines?

The physical and vaccines are separate services. If school enrollment requires missing vaccines, we give flu and Tdap at the same visit; for the rest we tell you where to get them.

How much does a school or sports physical cost?

The exam includes the signed form. Ask for the price when you arrive or call us at (832) 530-4188: direct payment by cash or card, no insurance needed. Demand is high in July and August: come early to avoid waiting.
